26 year old male patient suffered a stab injury with knife in his left side of chest. He was admitted to Metro Hospital, Faridabad emergency department with severe difficulty in breathing and abdominal distension. CECT abdomen & chest was done which showedmassive collection of blood in left side of chest (Haemothorax) with mild collection of blood in abdomen. Immediately chest tube was inserted in his left side of chest, which drained about 700 ml of blood.
After the initial procedure patient was stable overnight, his respiratory distress was settled. Next morning, patient complained of abdominal pain with abdominal distention, Blood investigations and repeat CECT was done, which showed fall in hemoglobin and increased hemoperitoneum. After all investigation and evaluation Dr. Mradul Garg, Sr. Surgeon of Metro Hospital, Faridabad along with Dr. Sachin Mittal planned a diagnostic laparoscopy (inspection of abdomen by laparoscopy).
During procedure about 3-4 liter of blood was present in abdomen with a tear in the left side of diaphragm. The source of bleeding was controlled and lavage done laparoscopically. Repair of diaphragmatic tear was also repaired laparoscopically.
Dr. Mradul informed that “the highlighting aspect of this surgery is that, normally these kind of operations are managed by open surgeries in which long incision of about 20-25cm is given and includes severe pain & discomfort, respiratory infection, wound infection, increased hospital stay and ultimately increased cost. By laparoscopy technique, only 4-5 small incision of less than 1cm was given which avoided all above complications of open surgery.
