Stroke is the third commonest cause of death and leading cause of adult disability in India. Recently the thrombolytic agent (rt-PA/ Alteplase), received a license for the treatment of acute ischemic stroke within 4.5 hours of symptom onset. The benefits of thrombolysis can be substantial. By promoting the rapid recognition of Stroke symptoms in the community, prompt transport of patients to specialized stroke units for early investigation, Neuro imaging, stabilization and treatment there would be substantial net benefit for all patients with stroke. Clearly we have the long way to go before BRAIN ATTACK is given the same degree of priority and urgency as heart attack currently is. We have successfully thrombolysed more than 50 patients.
Thrombolytics restore cerebral blood flow in patients with acute ischemic stroke and results in addressing the neurological problems.
The majority of strokes are due to blockage of an artery in the brain by a blood clot. Prompt treatment with thrombolytic drugs can restore blood flow before major brain damage has occurred. Successful treatment could mean that the patient is more likely to make a good recovery from their stroke.
Mechanism of Thrombolysis.
Intravenous Thrombolysis has become an important part of the Ischemic Stroke Management. For patients who meet criterias for IV Thrombolysis, research has shown good results of it in all subtypes of Ischemic Stroke.
Conversion of Plasminogen to plasmin leads to fibrinolysis and destruction of clot. This theoretically results in revisualization of a previously occluded blood vessel and reversal of brain Ischemia .

We are doing Thrombolysis in stroke patient on regular basis at our Neurology Department which is headed by Dr. Rohit Gupta, MD (Gold Medalist), DM(Neurology) ), Sr. Consultant- Neurology. We have thrombolysed more than 150 patients with excellent results. We have Thrombolysed difficult cases like patient with accelerated HTN, Patient on double antiplatelets with chronic renal disease rare case of protein C deficiency, patients on patient with 75 years of age.
Our centre has appropriate Neurological and Neuro-imaging resources that are capable of administering treatment with quick response. The centre is facilitated by prompt CT scan and MRI facility. We have specialized Neurolologist, Dr. Rohit Gupta, who has demonstrated expertise in Neuro-Radiology who interprets head CT scans to determine whether to administer thrombolytic agents to stroke patients.
The strategies to increase the use and awareness of thrombolysis therapy should include:
- Equipping the practicing physicians in the community with the practical knowledge of stroke management, including the importance of window period and the benefits of thrombolysis therapy.
- Educating the public about stroke warning symptoms, risk factors, the mortality and dependence associated with stroke and importance of time window for Thrombolysis treatment.
